In-home health monitoring

Keeping health care at home

Aurora Visiting Nurse Association Home Health Monitoring Program provides you with the best of both worlds: the comfort and independence of staying at home, and the security of daily monitoring and proper medical attention.

How it works

The system consists of a compact monitor and scale, which is placed in your home.

In just three minutes, the system collects a variety of vital signs such as heart rate, blood pressure, oxygen saturation, body weight and temperature every day.

The data is sent via the phone line to the Aurora VNA office, where a nurse reviews the information 365 days a year and responds appropriately. If there is a change in your health status our nurse contacts your physician to promptly address your needs.

Vital signs measurements

Every day, the Home Health Monitor measures:

  • Heart rate
  • Blood pressure
  • Weight up to 500 lbs.
  • Oxygen saturation
  • Temperature, if needed

In addition, personalized health-related questions allow for a comprehensive assessment of your status.

How much does it cost?

  • Medicare-eligible patients who qualify for home health care services and meet the selection criteria will be monitored at no cost.
  • Patients who do not qualify for home health services have the option of being monitored for a monthly fee.
